Pain Management Comprehensive Services:

• Cervical and lumbar epidural steroid injection.
• Selective nerve root and facet joint blocks.
• Major joint (hip, knee, shoulder), TMJ, intercostal, trigger point and sacroiliac joint injections.
• Celiac plexus hypogastric block.
• Lumbar sympathetic block.
• Greater occipital nerve block.
• Stellate ganglion block.
• Lysis of epidural adhesions.
• Radiofrequency Ablation Facet and Sacroiliac joint pain.

Referral Process:

• Referrals are accepted from all health care providers including, but limited to, surgeons, primary care physicians, chiropractors, occupational health, neurologists and oncologists.
• Self referrals by patients are not accepted.
• All patients must have a primary care physician prior to being seen at FRPMC. The referring or primary care physician will continue to be responsible for all medication prescriptions, including pain medications.
• Referring providers' offices must subit a completed referral form and all pertinent information. Documentation should include the reason for referral. Referral requests may be mailed or faxed to our office.

 All referrals are reviewed by our physician:

• The patient will be sent a questionaire and information about FRPMC. Questionaire must be completes prior to appointment. A postage paid envelope will be provided for returning your questionaire.
• Patients will be notified by phone of their appointment date and time. A nurse will instruct the patient about their medications, preparation, and the planned procedure.
• Referring health care providers and the primary care physician of record will be sent a copy of procedure report, recommendation of treatment or summary of care after each visit.

Diagnostic and Therapeutic Procedures:

•  All of our diagnostic and therapeutic procedures are performed on an outpatient basis, which means that patients are typically in our facility for less than four hours.
• Fasting is required six hours prior to the procedure.
• Treatment usually requires two or more visits.
• All patients must have transportation arrangements made for the day of their procedure. Public transportation may be used only when accompanied by an adult.
• Most people are able to return to work or resume their normal activities the following day.

Payments:

Our services are covered by most insurance plans. Patients or physician offices are welcome to contact our billing office with any questions regarding financial arrangements at 530-751-4771 or 530-751-4800
