FRHG Hospice Video

Fremont-Rideout Hospice, a member of Fremont-Rideout Health Group, is a non-profit organization of volunteers Hearts of Gold and professionals serving terminally ill patients and their families, primarily through home care. Watch the video.
The service area covers the greater part of Sutter and Yuba Communities. We honor a philosophy of care for terminally ill patients and their families. Symptom management and pain control, with spiritual and emotional support, allow a dignified death at home.

Hospice makes it possible for people to remain at home during much or all of their illness. Regular visits from professional staff teach the family how to provide the necessary care and are available 24 hours a day to assist both patient and family with the problems of the illness. Care is adjusted as needs change and continues on to help family members after the death.

Who Are the Care Givers?

We recognize that no one person can meet the needs of a patient with a terminal illness. As needed, we mobilize a diverse group of care givers including:

  • The patient’s family & loved ones
  • Physicians
  • Registered Nurses
  • Home Health Aides
  • Medical social worker
  • Hospice volunteers
  • Chaplain
  • Physical, occupational & speech therapists

What Does Fremont-Rideout Hospice Provide?

Nurses are on call 24 hours a day. We serve patients in the greater part of Sutter and Yuba Communities. Hospice is Medicare certified.

Who is Eligible For Fremont-Rideout Hospice?

Admission criteria for Fremont-Rideout Hospice include:

  • The patient must be terminally ill and have a prognosis of 6 months or less.
  • Patient and family are no longer pursuing curative treatment, but desire comfort measures only.
  • The patient, family and primary physician must be willing to participate and share a common understanding and expectation of hospice care.
  • The patient must reside within the geographical boundaries of the service area.

Individuals who participate in Medicare Part A are eligible for our Medicare Certified Program.

Who Pays For The Service?

Fremont-Rideout Hospice is a Medicare certified program. Private insurance may also have a hospice benefit. Fremont-Rideout Hospice accepts assignment of benefits from Medicare, Medi-Cal and some private insurance.<
